  1. Your Surveyor's Registration Number.
  2. Your employer or company trading name.
  3. A willing witness over 18 years of age to sign the deed.
  4. Eligibility as one of these business entity types:
    a.  Individual surveyor, licenced in your name or trading name
    b.  Company agent, representing a licensed surveying company
    c.  Company secretary of a licensed surveying company
    d.  Company director of a licensed surveying company
